Financial Policy

We make every effort to keep down the cost of medical care while maintaining the highest quality of service. As a courtesy to our patients, our offices participate with most insurance companies and managed health care programs. We submit claims once based on information you provide, so please be sure that you keep us informed of any changes to your insurance coverage.


If you are experiencing problems with your insurance company covering services, we suggest that you contact the Benefits Department of the employer providing the insurance for assistance. Patients are responsible for paying any deductible, co-payment, or non-covered charges at the time of the visit.

INSURANCE:

We bill participating insurance companies as a courtesy to you. You are expected to pay your deductible and copayments at the time of service. If we have not received payment from your insurance company within 45 days of the date of service, you may be expected to pay the balance in full. You are responsible to be sure all charges are paid whether by you or by your insurance carrier.


We do not bill secondary insurance companies.


Your time of service receipt includes all information necessary for submitting claims to your insurance company.

REFUNDS:

Patient/guarantor credits in amounts less than $20.00 will be retained on account to be credited toward future balances unless a written request for refund is received. Amounts $20.00 and greater will automatically be refunded to the patient/guarantor.


MANAGED CARE/COMMERCIAL INSURANCE:

If you are enrolled in a managed care insurance plan (i.e., HMO), you must receive a referral from our office before seeing a specialist. Retroactive referrals are not guaranteed. Patients must present a current insurance card for every visit.


MEDICAID:

PACGPA requires that you present your insurance card for every visit. For Managed Care plans such as Amerigroup, Aetna Medicaid, Cooks Children’s Health Plan, Parkland, Unicare, and others, you must choose one of our physicians as your Primary Care Provider. If we are not the PCP at time of service, you will be required to pay for the visit or reschedule the appointment.


MISSED APPOINTMENTS/LATE CANCELLATIONS:

Broken appointments represent a cost to us, to you and to other patients who could have been seen in the time set aside for you. Cancellations are requested 24 hours prior to the appointment. We reserve the right to charge for missed or late-canceled appointments. Excessive abuse of scheduled appointments may result in discharge from the practice.
